PROGRESS REPORT - Within 6 months of grant funding: Using the form below, provide a brief overview of the program and a summary of the progress to date.
FULL REPORT - Within one year of grant funding: A second and more detailed grant report is due and can be found here.

Provide a brief overview of the program and a summary of the progress to date. Please include activities and impact the program is having on the target audience and what amount of the grant funding has been expended. This overview and summary should be on your organization letterhead and be no longer than 500 words.
